In-house it was MACRO-10. This should have been a very strong clue and I still don't understand how this aspect of non-thruput got ignored.

Yup. I don't think we ever understood that aspect of computing.

What DEC had to do was do the preliminary work to make that transition as invisible to the customers as possible. The project that should have been done after SMP on both PDP-10 OSes should have been Bob's SIMH project so that all DEC customers wouldn't have to retrain their control files and procedures just because new gear and a new OS came into the door. The transitions the customers would have to make to new gear-OS could be done as their development work evolved. They wouldn't have had to stop all of their projects just to adapt to the new gear that DEC made.

ROTFL. A physicist told me the story about gear that wouldn't work well. They finally tracked down the problem to a radio rock station emission.
