Sometimes (that's actually paranoid schizophrenia--there are other types). It's also indicated by paranoid delusions, cluttered thoughts, and a few other indicators. There are more mild forms without the paranoid delusions or hallucinations, as well.

Treating it is difficult, since many times they will simply go off their medication. And AFAIK there is no treatment for catatonic schizophrenia.

Again, that refers only to paranoid schizophrenia.
